PARTICIPANT WAIVER & RELEASE - MINDFUL EXPERIENCE CLASS

  1. Assumption of Risk
    I acknowledge that I am voluntarily participating in the above-described mindful experience class. I understand that participation involves some inherent risks, including but not limited to slips, trips, minor cuts or burns from art and other class materials, allergic reactions, or other accidental injury. I accept and assume all risks associated with my participation.
  2. Release and Waiver of Liability
    To the maximum extent permitted by law, I release, waive, discharge, and covenant not to sue Jennifer Showers/LifeDesignLady, instructors, employees, agents, volunteers, and the venue and their respective owners and affiliates (collectively, “Released Parties”) for any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death, whether caused by the negligence of the Released Parties or otherwise, that may occur as a result of my participation.
  3. Indemnification
    I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs (including reasonable attorneys’ fees) they may incur due to my negligent or intentional acts or omissions while participating.
  4. Medical Treatment
    If I am injured or become ill, I authorize the Released Parties to seek emergency medical treatment on my behalf if they reasonably believe it is necessary. I understand I am responsible for any medical costs incurred.
  5. Property Damage
    I am responsible for any damage I cause to the venue, equipment, or materials beyond normal wear and tear and agree to reimburse reasonable repair or replacement costs.
  6. Acknowledgement of Understanding
    I confirm that I am at least 18 years old, have read this Waiver & Release, understand its terms, and sign it voluntarily. I understand this document affects my legal rights.
